Registered Nurse Assessment Coordinator (RNAC / MDS Coordinator) Oak Village Healthcare Center | Oaktown, IN Part-Time (20 hours a week) Oak Village Healthcare Center is seeking a compassionate and detail-oriented Registered Nurse Assessment Coordinator (RNAC / MDS Coordinator) to join our dedicated skilled nursing team in Oaktown, Indiana. This part-time position offers an opportunity to make a meaningful difference by ensuring accurate resident assessments, regulatory compliance, and high-quality, individualized care planning. Your Role & Responsibilities As the
RNAC / MDS
Coordinator, you will: Conduct routine resident assessments according to established schedules and as needed for condition changes, hospital returns, and other clinical events Accurately observe, assess, and communicate changes in resident condition to the interdisciplinary team Collaborate with Social Services, Activities, Dietary, and Nursing departments to ensure timely and accurate completion of assessments and Care Area Assessments (CAAs) Complete MDS sections, quarterly and annual assessments, CAAs, and care plans in compliance with state and federal regulations Develop, update, and monitor individualized resident care plans based on assessment findings Prepare and distribute the MDS schedule to appropriate team members Transmit MDS data to the state per required timelines and maintain organized, accurate documentation What We're Looking For Active Registered Nurse (RN) license preferred; LPNs with strong MDS experience will be considered Minimum of 5 years of experience as an
RNAC / MDS
