Job Description

Rehabilitation Support Services, Inc. is hiring a full time Registered Nurse (RN) to work in our residential programs in Orange and Sullivan counties and our Sullivan County PROS (Personal Recovery Oriented Services) program. Qualifications License and current registration as a Registered Nurse in the State of New York An ideal candidate would be a graduate of an accredited program of professional nursing and have at least one year of experience as a Registered Nurse with individuals with psychiatric in a community-based health services setting CPR Must demonstrate excellent written and oral communication skills. Ability to handle multiple priorities and work as a member of a team. In Residential Services The qualified candidate will be responsible for duties including, but not limited to conducting health assessments, monitoring and coordinating medication, providing staff, client and family education, coordinating medical care, and providing crisis intervention services. In PROS The Registered Nurse (RN) in our PROS (Personalized Recovery Oriented Services) Program will play a vital role in supporting individuals with mental health challenges by facilitating health-related education, conducting health assessments, and collaborating with medical providers. This position also includes providing individual health teaching and collaborating with providers to ensure a comprehensive and supportive environment for all program participants.
Key Responsibilities:
In PROS:
Facilitate health-related educational groups that focus on wellness, medication management, and health promotion for individuals in recovery. Provide individualized health teaching to program participants, addressing topics such as chronic disease management, medication adherence, and general well-being. Conduct comprehensive health assessments for program participants, evaluating vital signs, medical histories, and physical health needs. Collaborate with the clinical team to address health concerns that may impact mental health treatment or recovery. Coordinate and collaborate with medical providers, including physicians and specialists, to ensure a holistic, integrated approach to patient care. Communicate patient progress and concerns effectively within the multidisciplinary team. Complete all required documentation for patients, including monthly summary notes, individual session notes, and health assessments. Ensure all documentation is accurate, timely, and compliant with organizational policies and regulatory standards.
In Residential:
Performs clinical/physical assessments and triage Gives medications and treatments as indicated Takes and records vital signs as needed. Participates in and/or directs the rendering of first aid and emergency care as needed or directed Assessment by direct observation on consumer health status /Evaluation of physical status changes for exacerbation of existing conditions, onset of disease, and/or responses to care and treatment Communicate with physicians, families, interdisciplinary team members and clinicians involved in providing services to individuals Communicate and follow up on consumer hospitalizations, emergency room visits, reports of injury, trauma, accidents, illness, distress and other health issues as individually required Accompany individuals on medical appointments when necessary to advocate for and discuss health issues Communicate with team members as to findings In urgent/emergent situations, you may receive phone calls outside of routine working hours regarding individuals in residential services.
  • Conducts training for staff in medical administration, personal hygiene, basic first aid, dental hygiene, infection control, adaptive equipment, vital signs, signs and symptoms of illness and other in-services as may be necessary to meet needs of individuals in each program.
  • Provide training to residents about the importance of medication adherence, personal hygiene, basic first aid, infection control, signs of illness, and illness/symptom management.
  • Participates in interdisciplinary team meetings and bi-annual reviews as required by each program.
Arranges medical services for supported individuals Provides care direction as needed to staff based on the individual's needs Utilizes principles of infection control when caring for supported individuals who are residents in our programs. Coordinate discharges from facilities as needed to transitions residents back to their housing program. Documentation and follow up of all findings Limited on call responsibilities. Perform other Duties as assigned

EOE Pay:
