Job Description
Registered Nurse Victoria, TX 77902 Part-time, Contract Part-time, Contract Position Summary Americare Nursing Services is seeking a compassionate, skilled, and dedicated Registered Nurse (RN) to provide high-quality nursing care to patients in their homes. The RN is responsible for assessing patient needs, developing and implementing individualized care plans, coordinating with physicians and interdisciplinary team members, and ensuring compliance with all applicable federal, state, and agency regulations. The ideal candidate will have strong clinical judgment, excellent communication skills, and a commitment to delivering patient-centered care that promotes independence, dignity, and quality of life. Key Responsibilities Perform comprehensive nursing assessments of patients in their homes. Develop, implement, and update individualized care plans based on patient needs and physician orders. Administer medications, treatments, and skilled nursing procedures as prescribed. Monitor patient condition and response to treatment, documenting all findings accurately and timely. Provide education to patients, families, and caregivers regarding disease management, medications, treatments, and safety precautions. Coordinate care with physicians, therapists, social workers, home health aides, and other interdisciplinary team members. Communicate significant changes in patient condition to the physician and clinical supervisor promptly. Ensure compliance with Medicare, Medicaid, state, and agency regulations and standards of practice. Complete OASIS assessments, recertifications, resumptions of care, discharges, and other required documentation as applicable. Participate in case conferences, care coordination meetings, and quality improvement activities. Supervise and delegate care responsibilities to LVNs, home health aides, and other assigned staff as appropriate. Maintain patient confidentiality and protect all health information in accordance with HIPAA regulations. Promote a safe environment for patients and caregivers by identifying risks and recommending appropriate interventions. Participate in on-call rotation as required. Qualifications Current and unrestricted Registered Nurse (RN) license in the State of Texas. Graduate of an accredited school of nursing. Minimum of one to two years of nursing experience preferred; home health experience strongly preferred. Current BLS/CPR certification required. Knowledge of home health regulations, Medicare Conditions of Participation, and OASIS documentation preferred. Valid Texas driver's license, reliable transportation, and current auto insurance. Ability to travel to patient homes within assigned service areas. Strong clinical assessment, critical thinking, and organizational skills. Excellent verbal and written communication skills. Ability to work independently and manage time effectively. Physical Requirements Ability to stand, walk, bend, stoop, kneel, and lift up to 25 pounds as needed. Ability to travel frequently to patient residences. Ability to use standard medical equipment and electronic documentation systems. Work Environment Primary work setting is in patients' homes and community-based environments. Exposure to varying home conditions, pets, and environmental factors.
