Job Summary We are seeking a highly skilled and detail-oriented Registered Nurse / MDS Coordinator to oversee the comprehensive assessment and care planning processes within our facility. The ideal candidate will possess extensive clinical expertise, including proficiency in medical documentation, coding, and utilization management, to ensure optimal patient care and regulatory compliance. This role offers an opportunity to lead clinical documentation improvement initiatives while collaborating closely with interdisciplinary teams to enhance resident outcomes. Duties Conduct thorough Minimum Data Set (MDS) assessments in accordance with federal and state regulations, ensuring accuracy and completeness. Coordinate interdisciplinary team meetings to develop individualized care plans aligned with residents' needs and preferences. Review medical records, documentation, and coding (including
ICD-10, CPT, ICD-9
) to support accurate reimbursement and compliance with Medicare and Medicaid standards. Utilize electronic health record (EHR) systems such as Epic, Point Click Care for documentation review and data entry. Lead utilization review and management activities to optimize resource use while maintaining quality standards. Collaborate with clinical staff on discharge planning, hospice care, and post-acute services to ensure seamless transitions of care. Maintain up-to-date knowledge of NCQA standards, HIPAA regulations, and other compliance requirements related to medical records and documentation. Provide training and support to staff on medical terminology, coding practices, and documentation improvement strategies. Qualifications Valid Registered Nurse (RN) license with current licensure in the state of employment. Proven experience in nursing within inpatient settings such as hospitals or long-term care facilities; ICU or trauma center experience is highly desirable. Strong knowledge of MDS processes, Medicare regulations, ICD-10/ICD-9 coding, CPT coding, and DRG reimbursement methodologies. Familiarity with EMR/EHR systems including Epic, Cerner, Athenahealth, or eClinicalWorks. Experience in utilization review, case management, or clinical documentation improvement initiatives. Excellent understanding of physiology, anatomy, medical terminology, and healthcare compliance standards such as HIPAA. Demonstrated ability to lead multidisciplinary teams effectively while maintaining attention to detail in documentation review. Prior experience working with managed care organizations or in outpatient settings is a plus; nursing home or home care experience is preferred.
