As a Case Manager, you will play a vital role in supporting patients on their healthcare journey by working closely with providers to deliver high-quality primary care services. This position focuses on chronic disease management, patient education, preventive care, and empowering individuals to take control of their health and wellness. Position Responsibilities Include:

Assisting providers in the delivery of primary healthcare services Providing health education and chronic disease management support Educating patients on diabetes care and preventive health services Coordinating care and serving as a case manager for patients with chronic conditions Performing designated nursing procedures within the Arkansas State Board of Nursing scope of practice ✅

Qualifications:
