• $133,000 per year Overview VITAS Healthcare is seeking an experienced and compassionate RN Team Manager to join our hospice leadership team in Lathrop, California .

This leadership role is responsible for overseeing patient care services, interdisciplinary team coordination, staff supervision, quality assurance, and operational excellence to ensure exceptional end-of-life care for patients and families. This position operates out of our Lathrop, CA office and follows a Monday through Friday schedule , with participation in an on-call rotation as needed.

Competitive compensation package Health, dental, vision, life, and disability insurance 401(k) with generous company match Paid Time Off (PTO) Tuition reimbursement Flexible spending accounts (FSA) Employee Assistance Program Legal insurance Cancer and critical illness coverage Roadside assistance Career advancement opportunities Key ResponsibilitiesPatient Care Leadership Direct all patient care services for the interdisciplinary hospice team Ensure care plans are integrated and aligned with patient/family goals Review admissions, recertifications, and discharge planning Collaborate with physicians regarding pain and symptom management Manage team scheduling and coordinate 24/7 patient care coverage Facilitate interdisciplinary team meetings and plan-of-care updates Participate in on-call rotation Staff Management & Supervision Interview, hire, train, supervise, and evaluate clinical team members Conduct field visits and provide mentorship to staff Support documentation accuracy and clinical excellence Monitor productivity, scheduling, and territory assignments Approve payroll, mileage, and related expenses Quality & Regulatory Compliance Ensure compliance with VITAS policies and regulatory standards Conduct chart audits and utilization reviews Lead quality improvement initiatives Participate in surveys, audits, and annual program reviews Monitor patient and family satisfaction outcomes Customer Service & Community Relations Resolve patient/family concerns and service issues promptly Build strong relationships with physicians, facilities, and referral partners Participate in customer in-services and community outreach efforts Visit LTC and contracted facilities to ensure quality coordination of care Financial & Operational Oversight Monitor resource utilization and patient care costs Support productivity and budget management goals Approve patient care-related invoices and expenditures Qualifications Current and valid Registered Nurse (RN) license in California Minimum 2 years of supervisory experience in a healthcare setting Strong understanding of hospice, palliative care, and interdisciplinary care coordination Excellent leadership, communication, and organizational skills Reliable transportation with valid driver's license and insurance Working knowledge of EMR/documentation systems preferred

Preferred:
