Registered Nurse (PRN) Palmyra , PA 17078 Apply Overview Salary Range $39.00 - $42.75 Hourly Apply Description About the Position The Registered Nurse coordinates and ensures quality nursing care is provided to elders. They also instruct Nursing Assistants on medical care and issues. Essential Duties and Responsibilities Performs various duties in order to ensure quality nursing care is provided to elders. Ensures that care provided is focused on maintaining or attaining the highest practical level of functioning as illustrated by the following: Evaluates elders condition and analyzes information obtained in order to develop appropriate interventions to prevent decline. Records signs of change in condition and notifies appropriate staff per facility protocol to evaluate weight loss, decline in ambulation, development of skin breakdown and other changes. Notifies physician of changes in elder's condition. Notifies appropriate staff of need for restorative nursing evaluation based on change in condition. Administers medications and treatments to elders as per physician order. Documents administration in electronic health record (EHR). Informs Physician of any adverse reactions to medication. Evaluates elders upon admission to develop baseline Care Plan and further assesses elder to contribute to comprehensive Care Plan. Attends and participates in Care Plan meetings as needed. Updates Care Plans as needed. Responds to emergencies; evaluates the condition of the elder; decides on the type of medical attention required consistent with elder's Advanced Directive; calls appropriate ambulance service if needed; notifies physician when a change in condition occurs; notifies family or Agent under a Power of Attorney when elder is transferred or admitted to the hospital. Confers with elders and families about care . Makes referrals to other departments when necessary. Ensures that care and services provided are accurately documented in the EHR and paper charts. Maintains elder clinical files and ensures charts are updated as needed. Ensures all required documentation is completed in a timely manner. Completes elder incident report forms and initiates interventions to prevent injury. Updates Care Plan as needed and informs on-coming shift during report. Ensures elders are aware of the services available to them. Encourages elders to participate in activity programs and provides staff support to enable elders to get to and from programs. Maintains accountability for controlled substances. Inventories controlled medications at the beginning and end of the shift with another licensed nurse from the off-going or on-coming shift according to facility policy. Immediately notifies supervisor of discrepancies in medication count. Follows procedures for elder charge items. Obtains report from off-going shift and in turn, gives report to Nursing Assistants. Communicates status of elders to on-coming staff according to established procedures. Communicates with physicians and transcribes, clarifies, and implements physicians' orders. Assists with orientation of new employees. Participates in assigned educational programs and competencies , including mandatory in-service training, annually and as needed. Follows established infection control policies and procedures and ensures shift staff compliance. Obtains continuing education required for nursing licensure. Performs other duties as assigned. Qualifications Current PA licensure as a Registered Nurse in good standing. Graduation from an accredited Nursing program and willingness to take PA state licensing board examination. Valid Graduate Temporary Practice Permit (TPP) issued by the PA State Board of Nursing. CPR certification or ability to obtain. Share job details to
