What You'll Do (The Fun Stuff): Lead in-home assessments and build individualized care plans Manage ongoing visits, teaching, and clinical decision-making Guide families through home recovery with confidence and compassion Collaborate across an elite clinical team for seamless patient care Document accurately and professionally (we'll train you on OASIS if needed)

What You Need:

Active MN RN license 3+ years RN experience (any setting—home health is a bonus!) Strong assessment + communication skills A reliable vehicle and a passion for independent nursing The confidence to lead care in the home setting
