Opens a new window. Position Summary We are seeking a compassionate and experienced Registered Nurse (RN) to coordinate resident care in a small, 10-bed hospice house setting. This role serves as a clinical leader within the home, focusing on assessments, care planning, staff support, and collaboration with families and healthcare providers. Responsibilities Coordinate and complete resident admissions and assessments Monitor and assess changes in resident condition and level of care Develop and update individualized care plans Evaluate appropriateness of admissions based on care needs and safety Coordinate with physicians and ensure all orders are obtained and current Update and maintain Medication Administration Records (MARs) Provide clinical oversight and competency checks for CNAs, Med Aides, and LPNs Serve as a clinical resource and provide guidance to staff Communicate with families, hospice agencies, physicians, and care teams Educate families on end-of-life care and expectations Coordinate care with hospice agencies and interdisciplinary teams Conduct clinical audits to ensure quality, compliance, and accurate documentation Ensure timely and accurate documentation by all clinical staff Monitor and support infection control practices Review and follow up on incidents, falls, and changes in condition Respond to and guide staff during changes in condition or emergencies Assist in maintaining clinical supplies and equipment readiness Participate in on-call rotation every other week Qualifications Active RN license (Iowa) Experience in hospice, palliative care, long-term care, or residential care preferred Strong clinical assessment and decision-making skills Ability to work independently and lead a team Compassionate, patient-centered approach Excellent communication and organizational skills Compensation & Benefits Hourly pay (based on experience) On-call stipend Supportive, team-oriented environment Meaningful work in a mission-driven, non-profit setting Why Join Us?
