Registered Nurse (RN) -
Field Case Manager Location:
Field-Based / Travel (Must reside in Illinois; primary coverage area includes Wayne County and the greater Macomb/McDonough County regions)
Compensation:
$46.66 per hour +
Mileage/Travel Reimbursement Licensure:
Active, unrestricted Illinois RN License required Role Overview As a Field Case Manager, you will play a pivotal role in advocating for members right in their communities. Moving beyond a desk, you will facilitate a collaborative process of assessment, planning, and advocacy through direct, face-to-face interaction. You will utilize clinical tools, localized data, and regional healthcare networks to evaluate member needs, optimize care plans, and ensure cost-effective, high-quality outcomes. This is an active, fast-paced role that relies heavily on clinical expertise, independent decision-making, and regional travel.
Key Responsibilities In-Home & On-Site Assessments:
Conduct detailed, face-to-face reviews of referred members' needs, home environments, and eligibility within the assigned territory.
Localized Case Resolution:
Determine the best approach for meeting member needs by building relationships with local healthcare providers, internal/external programs, and benefit plans.
Regulatory Compliance:
Apply and interpret standardized case management plans, state policies, and regulatory standards to ensure the appropriate administration of benefits.
Quality & Care Continuity:
Utilize established case management and quality processes in strict accordance with accreditation guidelines and company procedures to bridge gaps between inpatient care and home life.
Community Advocacy:
Serve as a mobile bridge between members and vital community healthcare resources to promote optimal health outcomes.
Qualifications & Experience Education/Licensure:
Must be a Registered Nurse with an active, unrestricted IL license.
Clinical Experience:
Minimum of 3 years of clinical practice (experience in home health, hospice, ambulatory care, or community health is highly preferred).
Industry Knowledge:
Experience in healthcare and/or the managed care industry.
Case Management:
Prior experience in Care Coordination or Field Case Management is highly preferred.
Technical & Digital Proficiency:
Comfort managing digital documentation, electronic health records, and communication tools on portable devices while in the field.
Required Skills Communication & Rapport:
Exceptional verbal and written communication skills for building trust with members in their homes and managing internal reporting.
Adaptability & Autonomy:
Ability to manage an independent travel schedule, multitask, and prioritize effectively in an evolving field environment.
Critical Thinking:
Strong ability to interpret complex criteria and guidelines independently.
Physical Requirements & Environment Setting:
Field-based role requiring daily travel to member residences, clinics, and community facilities across Wayne and Macomb/McDonough regions.
Physical Demands:
Requires driving, walking, navigating varied environments (such as private homes), and managing portable electronic equipment.

Pay Details:
