Resident Care Coordinator/RN

Egle Nursing and Rehab Center is now accepting resumes for the position of Resident Care Coordinator. This is a full time daylight managerial position. EOE Responsibilities includes managerial functions in addition to:

• Coordinates Resident appointments with transportation in conjunction with the Director of Nursing.
• Conducting GCNA Training Classes.
• Conducting CPR Training Classes.
• Oversees the Resident Weight Committee which meets weekly with the Dietary Manager, Dietician, ADON, Social Work, MDS Coordinator and Director of Nursing.
• Reviews Resident admission documentation for accuracy and performs admissions observations.
• Conducts review for accuracy of all Baseline Care plans and performs Baseline Care Plan Summaries.
• Makes Baseline Care Plan Summary according to due date designated by MDS Coordinator.
• Coordinates resident discharge planning instructions and appointments in conjunction with Social Work.
• Conducts follow up of Resident Concurrent Reviews and assesses whether appropriate action was taken.
• Oversees the Resident Pain Management Program in conjunction with the Nurse Manager.
• Oversees reduction of resident psychotropic drug usage in conjunction with the Consulting Pharmacist and Psychiatrist.
• Other duties as may be assigned by the Director of Nursing or Administrator.
