JOB SUMMARY
Reporting to the Health Services Director, the Resident Services Coordinator provides assistance to residents who may be in need of support or services to allow them to remain safely in their independent apartment as long as possible with dignity and the highest quality of life. Responsible for evaluating the psychosocial and functional needs of residents and provides direct services as needed. The Resident Services Coordinator respectfully interacts with all residents, family members, visitors, vendors, and staff. This is a safety sensitive position.
ESSENTIAL FUNCTIONS

Assists with pre-residency assessments of potential residents. Ensures confidentiality of all resident information and documentation. Maintains essential records on all residents served and documents activity as appropriate. Works with Wellness Nurse to identify residents in need of service, either by direct referral or by the regular review of incident reports to monitor and assess residents who may be at risk and make appropriate referrals to services if necessary. Assesses, intervenes, and works to resolve issues regarding residents' care and safety Approves and coordinates free meal delivery to residents too ill to come to the IL Dining Room. Provides psycho/social support to residents and their families and assist with transition process. Make condolence call or visit to spouse/family after death of an I.L. resident. Collaborates with Transitions of Care team to make the recommendation for a higher level of care and refer to Wellness Nurse to initiate transition process. Assists with the facilitation of transfer by means of coordinating with staff and communicating with families. Provides information and support to the resident and family to ensure a smooth transition and timely clearing of IL apartment. Conducts follow up assessments within two (2) weeks after move-in to determine level of adjustment to new home and community surroundings. Educates and encourages the importance and assist with facilitation of Advance Directives, i.e., Living Will, Health Care Surrogate, Durable Power of Attorney documents and facilitates completion of such documents.
Other documents/information to update:
HIPAA release form, and update emergency contact information. Data entry and update into electronic records. Participates in the new resident Meet and Greet meetings to educate new residents onboarding the function, protocol and resources provided by the Resident Services Department. Monitors security and incident reports with Wellness Nurse and provide follow-up as needed. Responds to weekend/after hour deaths/emergencies when necessary. Manages and monitors Outside Service Providers to ensure registration and compliance with Outside Service Provider's policies and procedures. Obtains and maintains referral network within the community for available services, make appropriate referrals, and consult with other professional services as needed. Attends and participates in supervisory meetings, weekly Transitions of Care meetings, and other committees and meetings as needed. Collaborates effectively with internal departments and all levels of personnel on a wide variety of resident issues and provides education to staff. Continues to enhance professional skills through continuing education, community services, leadership training, and other professional activities. Share knowledge gained from training and educational services. Review resident policies with residents for educational purposes as well as non-compliance as required. Conducts annual Rules of the Road for EMV's with Security Department. Ensure EMV registration for all residents. Conducts annual Rules of the Road for EMV's with Security Department. WORKING CONDITIONS
Work is conducted primarily in health care facility. There is possible exposure to communicable diseases and blood and body fluids that may contain HIV and/or HBV. This position frequently requires working within strict deadlines, under stressful conditions and with a variety of personalities, who may at times be emotional.

MINIMUM QUALIFICATIONS
Bachelor's degree from an accredited college or university required, master's preferred 5+ years of related experience and/or training in geriatrics in long term care, home health or other health care setting or community-based geriatric case management required Licensed social worker or LPN, preferred Computer literacy which includes, but is not limited to, Microsoft Office Suite
