Job Description
Residential Care Provider (RCP) PT
Carry out a person-centered Individual Service Plan (ISP).Ensure individuals are dressed neatly and appropriately and are always groomed.
Assists and encourages individuals to care for immediate personal needs such as toileting, washing their hands, and eating.
Teaches individuals daily skills such as household chores, laundry, cleaning, and washing dishes, as appropriate according to their individual plan.
Transports individuals to work, day programs, doctor appointments, shopping, recreational activities.
Assist with scheduling medical appointments.
Assists in the implementation of individual and behavioral plans and provides feedback for plan goals.
Assists individuals in the pursuit of recreational, social and community activities based on their preferences and individualized treatment plans.
Assists and encourages individuals in the establishment of social networks.
Promotes participation and involvement in community outings.
Ensure each individuals rights are protected.
Treats individuals with dignity and respectMaintains the residence in a clean and organized condition.
Other duties as assigned.
Certified Nursing Assistant certification (CNA), Direct Support Professional (DSP), Direct Care Professional (DCP) or Patient Care Technician (PCT) preferred.
Must have a valid Drivers License and a clean Motor Vehicle Record in accordance with company policy.
Must pass a criminal background check (including fingerprinting).Requires TB Evaluation at pre-employment and annually.
Must be able to lift 20 pounds.
Requires long periods of sitting/standing.
Must have the following technical skills Word, Excel and Electronic Health Record (Care Logic).Must be willing to transport residents in the company van.
Must demonstrate the ability to maintain professional boundaries with clients in a residential treatment setting.
Must be able to lift and carry groceries and supplies and move furniture for cleaning.
Must be able to assist disabled individuals on/offvehicle and secure seatbelts.
Must be able to use a gait belt and mechanical lift to lift and position individuals, as needed. Oxford Place The Residential Care Provider monitors general resident activity to observe and report behavior/progress of individuals with severe developmental disabilities living in a group home or other community residential setting and provides individualized skills training courses according to residents needs.Duties & Responsibilities:

Minimum Qualifications:
Must have high school diploma or GED AND six (6) months of relevant job-related training or experience.Note:
Must be 21 years of age, have valid Georgia Driver's License, and a clean motor vehicle report.Preferred Qualifications:
Experience working with clients with mental health and substance abuse diagnosis in a residential settingFlexibility in managing daily tasks is of utmost importance.Certified Nursing Assistant certification (CNA), Direct Support Professional (DSP), Direct Care Professional (DCP) or Patient Care Technician (PCT) preferred.
