Job Description

Job description Legacy of Brownsdale has earned a reputation for offering extraordinary health care supervision to our 32 assisted living residents. We are seeking an RN who will help us continue those same levels of service. Are you that extraordinary person? Would you like to be a core part of a true team? Would you like to be supported by your coworkers and the owner of the company? If so, you may be our person! This role is a FT position (hours are flexible) that pays $43.27 to $57.69 hourly. No weekends, and 6 holidays per year. Every 2 weeks this position has Phone Triage hours associated with it for resident care related issues. You will also have the support of a full-time LPN, a Health Unit Coordinator, a full-time on site LALD and a fully staffed, well-trained and well-qualified direct care team. This position is not responsible for staff scheduling. Home Health, Long Term Care ,or Hospice experience will benefit you in this role.
JOB SUMMARY
The RN will manage the day-to-day healthcare operations of the community to ensure resident's healthcare needs are met. Ensure residents are treated with respect and dignity and ensures quality care as resident's healthcare needs change. Responsible for the direct supervision of care giving and coordination of health care needs within the community. Provide training, supervision, and monitoring of associates. As a representative of Legacy, all comments, attitudes, actions and behaviors have a direct effect on the facilities image and perceptions of quality service. Interactions with residents, families, physicians, referral sources, visitors, co-workers, supervisors, vendors, etc., must be in a manner that is friendly, supportive, courteous, respectful, cooperative and professional. This behavior should promote an atmosphere of teamwork, which is congruent with the facility standards and guideline to promote positive relationships. Report to the Facility Manager/LALD.
PERSONNEL
Assist in the interviewing and hiring of LPN's and CNA's. Develops or assists in development and implementation of plans for nursing care. Participate in annual employee reviews. Coordinates and participates in the employee corrective actions that affect healthcare. Conducts supervisory visits of staff, as required by law. Assures optimal quality of care is provided in a safe environment. The training of staff as required by the State of Minnesota. Oversee Medication Training of all staff. Coordinate with Administration to ensure that staff have met the training requirements of the State of Minnesota. Develop, implement and evaluate programs to promote the recruitment, retention and continuing education of the nursing staff Intervenes in crisis situations and investigates all unusual incidents. Responsible for scheduling LPN coverage within the community. Exercise sound judgment, using personal knowledge base, and management experience in decision-making and delegation of accountability responsibilities to others.
RESIDENT CARE
Assess each resident prior to move in to assure that the community can meet the needs of the potential resident. Conduct an assessment on each resident, quarterly, or if needs change. Provide Resident Care Plans according to the current assessment on each resident. Ensure that Resident Care Plans are current and being followed by direct care staff. Coordinate and participate in resident care conferences. Follow up and correct any issues regarding healthcare. Plan and facilitate meetings and committees to address patient care issues; proactively handle survey and/or standards of care issues. Provide direct resident care, including passing medications, as needed. Maintains contacts with individuals both within and outside of department who might impact nursing care. This includes the coordination of physical therapy and hospice services. Communicates effectively with residents and their families and at all levels of the organization. Report any neglect/abuse or maltreatment of a resident to the State of Minnesota. This includes any unexplained falls or injuries. Reports to the state include residents that have had medical treatment due to a fall. Coordinate and conduct an internal investigation and fax to appropriate agencies. Keep Executive Director informed of all reports to the State of Minnesota. Ensure that all charts are updated as needed. Including proper documentation in nursing notes. Ensure that all HIPAA forms are current and signed appropriately. Accountable for all admissions or discharges of potential residents or residents. Inform Executive Director of pertinent issues regarding resident care.
QUALIFICATIONS
Must be a currently RN licensed in the state of Minnesota or be able to transfer your out of state license to MN with minimal delay. Basic typing skills along with a working knowledge of personal computing and word processing software are required. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to effectively communicate in English and understand and follow written and oral instructions.
