Conduct in-home evaluations to assess needs and develop individualized care plans Lead service inquiries with families, using a sales-minded approach to show how our services improve lives Reassess care plans every 90 days, monitor client conditions, and oversee service activation in WellSky Provide weekly training and competency testing for our Care Professionals Act as the primary point of contact for families and a key collaborator with our staffing team•Your Benefits!
