RN Case Manager:
As an RN Case Manager, you will oversee and coordinate all aspects of resident care while supporting nursing staff across the campus. You'll play a key leadership role in ensuring quality care, regulatory compliance, and strong clinical outcomes.Key Responsibilities:
Coordinate and oversee resident care using the nursing process Supervise and support nursing staff (RNs, LPNs, TMAs, NARs) Perform resident assessments and respond to changes in condition Develop, update, and monitor care plans and clinical documentation Ensure compliance with state and federal regulations Participate in care conferences, audits, and quality improvement initiatives Collaborate with interdisciplinary teams, providers, and families Assist with admissions, resident evaluations, and care coordination Provide education, coaching, and support to nursing staff Step into direct care or emergency situations as neededWhat You Bring:
Required:
Active Minnesota Registered Nurse (RN) license Minimum 2 years of long-term care or related clinical experience Strong clinical assessment and decision-making skills Ability to work independently and manage multiple priorities Excellent communication and leadership abilitiesPreferred:
Experience with MDS/RAI processes Knowledge of Medicare guidelines Previous leadership or supervisory experience What Makes You aGreat Fit:
