Job Description

Responsibilities for the Registered Nurse Case Manager will include planning, organizing and directing hospice care. The selected candidate will have experience in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.
Qualifications:
Kansas RN License Experienced in similar position or oncology. Excellent communication skills Ability to work in a team environment as well as independently Travel to home sites and medical facilities requires current Driver's license along with proof of auto insurance and good driving record.
