The RN Case Manager will serve as primary contact person and coordinator for adults with disabilities in a group home setting in Berkshire County. Great supportive team environmentOpportunity to help people live their best livesProvide staff training and support on medical issues and techniques as neededPerform heath care assessments and develop written health care plans Consult with program staff on specific medical interventions, techniques and documentation requirements.
