What You'll Do Manage a hospice caseload across homes, assisted living, and skilled nursing facilities. Deliver pain and symptom management focused on comfort, dignity, and quality of life. Serve as the primary RN and case manager for patients and families. Provide education, support, and guidance through end-of-life transitions. Collaborate with physicians, social workers, chaplains, aides, and the interdisciplinary team. Coordinate care to ensure consistent, high-quality patient outcomes . Maintain accurate, timely EMR documentation and compliance with hospice standards. What We're Looking For Active Minnesota Registered Nurse (RN) license. Experience in hospice, oncology, ICU, palliative care, or home health . Strong clinical judgment and ability to work independently in the field . Excellent communication and relationship-building skills. Passion for patient-centered, end-of-life care . Reliable transportation and ability to travel throughout the service area.
