RN Director of Patient Services

The Gardens by Morningstar is a highly regarded Assisted Living facility located in a beautiful neighborhood setting in Oswego, NY. The position is a dream administrative position- Monday-Friday-No mandation with a highly competitive salary. We are looking for a highly motivated, team oriented RN to join our team of professionals. Job Duties include but not limited to: Conducts UAS assessments for all residents on a periodic basis and more often as needed Develops a plan of care for each resident and provides updates as necessary Oversees and conducts the weekly Service Plan/Fall meetings Serves as a liaison with home care agencies to assure resident care needs are being met Provides supervision of home health aide and personal care aide staffs Communicates on a frequent basis with residents primary care physicians to assure care needs are met Observes and provides recommendations on resident clinical issues/concerns. Serves as a liaison to residents and family members on clinical care issues
