Pay Range:

$2400 - $2500 weekly gross Responsibilities Provide skilled nursing care to patients in their homes in accordance with physician orders and care plans Perform patient assessments and monitor health status changes Administer medications, wound care, and other prescribed treatments Accurately document patient visits, care provided, and progress using Homecare Homebase (HCHB) Educate patients and families on disease management, medications, and home care instructions Coordinate care with physicians, therapists, case managers, and interdisciplinary team members Ensure compliance with infection control standards and agency policies Participate in care planning and patient progress reviews Deliver compassionate, patient-centered care while promoting safety and independence Requirements Active RN license Minimum 2 years of Home Health RN experience required Homecare Homebase (HCHB) experience required Current BLS certification Strong assessment, communication, and documentation skills Ability to work independently and manage patient schedules effectively Required Documents to Submit Your Profile To begin the onboarding and profile submission process, please provide: Updated Resume RN License BLS Certification Driver's License Skills Checklist Two Professional References Job Types Full-time Contract Benefits Referral program Work Location In person
