• Home Health Full Time & Part Time/ Per Diem to join our team!

You will be responsible for the assessment, diagnosis, and treatment of assigned patients in a home health setup. Let us know which areas you can service and we'll match you with the right referrals! Responsibilities Administration of prescribed medication, treatments, and therapies. Patient assessments and coordination of care Health, promotion, Providing nurse visits to home health patients. (home visits) Administer nursing care to ill, injured, or disabled patients Diagnose and establish patient treatment plans Monitor and report changes in patient symptoms or behavior Communicate with collaborating physicians or specialists regarding patient care Educate patients about health maintenance and disease prevention Qualifications Previous experience in nursing or home health Ability to build rapport with patients Strong problem solving and critical thinking skills Ability to thrive in a fast-paced environment Requirements Professional License Academic Diploma Updated CV/Resume with Experience Driver's License Auto Insurance Active BLS or CPR Certification Recent Physical Examination OR Fit-to-Work Note from Physician Valid (1 year) TB/PPD Test OR Valid (3 years) Chest Xray Covid Vaccine Card/Records (Includes Booster/s) Influenza Vaccine (or Declination) MMR Immunization Records Hepatitis B Immunization Records Professional Liability Insurance Photo (For Issuing of L&G Nursing Group ID) Reference Check (at least 2 credible referees) Got all the requirements needed?
