RN Manager / Health Care Practitioner Supervisor FirstLight Home Care - 3.6 Passaic County, NJ Job Details Contract 2 hours ago Qualifications RN License Interpersonal skills Bachelor of Science RN experience Driver's License Driving Full Job Description Position Summary The RN Manager / Health Care Practitioner Supervisor is responsible for conducting initial client assessments, developing and overseeing individualized Service Plans/Care Plans, and completing supervisory visits in clients' residences. This position provides clinical oversight of client care services, supervises clinical and caregiving staff, ensures compliance with state regulations and accreditation standards, and supports the overall operational success of the agency.

Certified Home Health Aides Assistants (CHHA) Qualifications A "Health Care Practitioner Supervisor" must meet New Jersey state requirements, including:
  • A New Jersey licensed physician; or
  • A Registered Nurse (RN) in good standing with: ○ A Bachelor of Science in Nursing (BSN) and at least two (2) years of combined public health nursing experience and progressively responsible professional nursing experience; or ○ Three (3) years of combined public health nursing experience and progressively responsible professional nursing experience.
To successfully perform this role, the individual must be able to carry out each essential duty effectively and professionally. The qualifications listed below represent the knowledge, skills, education, licensure, and experience required for the position. Essential Duties and Responsibilities
  • Oversees client services to ensure client needs are appropriately assessed and met.
  • Conducts initial client evaluations and ongoing reassessments of client needs.
  • Develops, implements, and updates individualized Service Plans/Care Plans and physician orders, when applicable.
  • Coordinates and supervises services in accordance with the established Care Plan.
  • Provides preventive, supportive, rehabilitative, and non-medical care guidance as appropriate.
  • Documents client assessments, supervisory visits, caregiver performance, client education, and staff training in accordance with agency policy and regulatory requirements.
  • Communicates changes in client condition or care needs to appropriate personnel, including physicians, caregivers, the Administrator, or Care Coordinator.
  • Observes, records, and reports changes in clients' physical, mental, or emotional conditions, including reactions to treatments or services provided.
  • Oversees limited skilled nursing services, when applicable.
  • Educates and counsels clients and family members regarding care needs, available services, and home care concerns.
  • Participates in quality assurance and performance improvement activities.
  • Ensures compliance with agency policies, state regulations, and accreditation standards, when applicable.
  • Oversees employee annual evaluations, competency assessments, and required training.
  • Coordinates staff meetings, client care conferences, and in-service education programs.
  • Supervises, mentors, and trains clinical and caregiving staff.
  • Assists clients with learning and implementing appropriate self-care techniques.
  • Participates in on-call rotation, including nights, weekends, and holidays, as required.
  • Assists with additional duties and special projects as assigned. Education and Experience
  • Bachelor of Science in Nursing (BSN) preferred or required per state regulation.
Minimum of:
○ Two (2) years of combined public health nursing and progressive professional nursing experience with a BSN; or ○ Three (3) years of combined public health nursing and progressive professional nursing experience as an RN.
  • Prior supervisory or management experience preferred.
  • Proficiency in Microsoft Office, Google Workspace, and scheduling or healthcare software systems.
  • Strong organizational, leadership, communication, and interpersonal skills required.
  • Ability to maintain a professional appearance and demeanor. Certifications, Licensure, and Registration
  • Current and active Registered Nurse (RN) license in the State of New Jersey.
  • Current valid driver's license and proof of automobile insurance. Language and Communication Skills
  • Ability to read, interpret, and understand instructions, correspondence, policies, and regulatory materials.
  • Ability to prepare reports, documentation, and professional business correspondence.
  • Strong verbal and written communication skills.
  • Ability to effectively communicate with clients, families, staff members, physicians, management, and the general public.
  • Ability to interpret professional journals, technical procedures, and governmental regulations. Reasoning and Critical Thinking Skills
  • Strong critical thinking and clinical decision-making abilities.
  • Ability to assess situations, prioritize responsibilities, and make sound decisions independently.
  • Ability to manage multiple responsibilities in a fast-paced environment.
  • Ability to work effectively under pressure and handle stressful situations professionally.

Specific vision abilities required include close vision, distance vision, peripheral vision, and the ability to adjust focus. Work is performed in office settings and clients' homes and may involve exposure to illnesses, infectious materials, and bodily fluids. The noise level in the work environment is typically moderate.
