Responsibilities:

The Registered Nurse - Value-Based Care (VBC) supports the organization's transition from volume-based to value-based reimbursement models by improving quality outcomes, reducing avoidable utilization, and enhancing patient experience. This role partners with providers, care teams, case management, population health, and payer partners to ensure clinical care delivery aligns with value-based contract performance metrics, including quality, cost, risk adjustment, and regulatory requirements. The Value-Based Care Nurse proactively manages assigned patient populations, with an emphasis on chronic disease management, care gap closure, transitions of care, and performance improvement initiatives tied to CMS and commercial value-based contracts.

Qualifications:

Education o High School Diploma, High School Equivalency (HSE), or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required Experience o 3 years of clinical nursing experience required Licenses and Certifications o RN - Current and valid Registered Nurse license to work within the state of California required o BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
