You'll help the Admissions Team review and manage referrals so future residents can transition in seamlessly, while also supporting the Nursing Team by coordinating admissions and discharges like a pro organizer. You'll be the go-to person for keeping nursing policies and procedures on track, making sure everything runs safely and efficiently. At the same time, you'll help lead and supervise the nursing staff while championing high-quality care for every resident. This position combines leadership, teamwork, organization, and compassion to keep residents cared for, staff supported, and the nursing department running like a well-oiled machine. What You'll Do Follows Residents' Rights policies at all times. Counsels certified nursing assistants and LPN charge nurses, updating the Director of Nursing as necessary. Attends all in-services and training sessions relevant to the position. Treats all information about residents, their condition and family as confidential.
Resident Admissions:
Check and review referrals in referral management systems. Collaborate withSW, DON, & NHA
re-admission referrals. Make pre-admission hospital visits & collaborate with hospital staff PRN. Prepare resident admission chart, review hospital d/c orders, enter admission orders in EMAR, complete admission nursing assessments, initial plan of care, etc.Resident Discharges:
Assess resident and primary CG needs for in-home care services, equipment, meds, and support post discharge. Collaborate with teamSW, PT, OT, ST
& Nursing regarding needs and communicate with TOC providers PRN. Schedule transition of Care appointments with PCP. Prepare d/c instructions, including teaching, written medication schedules, and TOC appointments and lab work. Arrange for needed med Rx's as needed at resident's pharmacy of choice. Communicate with all TOC providers at time of d/c. (fax PCP, & in-home care providers with d/c instructions and medication profiles.) Make post d/c follow up calls per SW direction.Social Services Support:
Assist Director of SW with resident care management & disposition needs per SW direction. Collaborate and network with referral providers, service providers, and community resources per SW direction. Utilizes clinical and leadership skills to 1) increase the quality of care for future, current, and discharged residents, 2) achieve a high level of resident and family satisfaction and 3) improve communication and teamwork (especially during the resident admission and discharge processes). Directs, evaluates, and supervises all resident care, initiating corrective action where necessary. Makes daily resident rounds and reports the status of nursing care to the Director of Nursing and Administrator. Supervises the implementation of nursing rehabilitation and restorative programs. Participates in the review of Quality Measure reports and the implementation of corrective actions to improve resident care. Assists with the analysis of incident and accident investigation reports to determine cause(s) and implements corrective action(s), when appropriate. Reviews residents admitted and compares physician orders with each resident's plan of care to assure appropriate implementation of the physician's plan of care. Participates in the Quality Assessment and Assurance Program for the nursing department, in cooperation with the Quality Assurance Committee. Participates in the interdisciplinary care plan team meeting and the weekly Medicare meetings. Maintains the confidentiality of resident and facility information. Identifies safety hazards and initiates corrective action where necessary. Assesses resident equipment and supply needs and makes recommendations to the Director of Nursing. Completes reports and assignments accurately and adheres to established time schedules. Establishes open lines of communication between all nursing personnel and consistently takes follow-up action as necessary. Observe infection control procedures relating to nursing services, including standard precautions. Responsible for the safety of residents under his/her care or supervision. Comes to work as scheduled and consistently demonstrates dependability and punctuality. Consistently works cooperatively with administration, nursing service personnel, physicians, community agencies, residents, families, and consultants. Comes to work in a clean, neat uniform and consistently presents an appropriate professional appearance. Accepts assigned duties in a cooperative manner. May be responsible to provide activities for residents in their household, conducting programs, visits and activities geared to the interests and abilities of their elders, documenting said activity participation and responses in the electronic medical record. Performs other duties and responsibilities as assigned. What We're Looking For Registered by the Commonwealth of Pennsylvania as a licensed registered nurse. Possesses initiative, judgment, dependability, and leadership skills. Possesses the ability to work cooperatively with others and can delegate wisely. Possesses the ability to teach skills necessary to provide quality care to the residents. Possesses basic computer data entry skills. Additional Information This position is Part-time 9:00am-5:30pm every Friday with on call every 7th weekend!
