RN/LPN Monday-Thursday NO WEEKENDS

Job Summary We are seeking a dedicated and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our healthcare team for a full-time position working Monday through Thursday, with no weekend shifts. This role offers an excellent opportunity to provide compassionate patient care across diverse clinical settings, including long-term care, outpatient clinics, and acute care environments. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories, to inform individualized care plans. Administer medications and injections accurately following established protocols, including IV infusions and immunizations such as botulinum toxin treatments. Assist with diagnostic procedures such as catheterizations, phlebotomy, and specimen collection while maintaining strict aseptic techniques. Provide wound care management, including ostomy care and dressing changes, ensuring optimal healing and patient comfort. Collaborate with interdisciplinary teams to develop discharge planning strategies and coordinate post-acute care for patients transitioning between settings. Utilize electronic health record (EHR) systems Engage in patient education focusing on medication administration, chronic disease management, pain control, and health coaching tailored to individual needs. Maintain compliance with HIPAA regulations and infection control standards while ensuring patient safety at all times. Skills Proficiency with EMR/EHR systems Experience in diverse clinical settings including long-term care, assisted living, hospital medicine, emergency medicine, or outpatient clinics. Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/ICD-10), and diagnostic evaluation procedures. Skilled in performing advanced nursing procedures such as IV insertion, management, tube feeding, and airway management. Geriatrics (MDS), behavioral health, hospice & palliative medicine experience. Ability to work effectively with individuals with disabilities or developmental delays across various age groups including pediatrics and geriatrics. Excellent communication skills for triage assessments, patient education, case management, and interdisciplinary collaboration.
