RN/LVN Field Nurse

Overview We are a home health agency seeking a compassionate and dedicated RN/LVN field Nurse to join our healthcare team. The ideal candidate will possess a strong background in nursing with experience in various medical settings. As a home health Nurse, you will provide high-quality patient care in a home setting, focusing on woundcare management, assessment, observation, medication management and teaching. Responsibilities Deliver comprehensive nursing care to patients in the field, including assessments and interventions. Assist with managing (setup pill boxes) medications and treatments as prescribed, ensuring adherence to infection control protocols. Perform wound care management as ordered by the Md Provide catheterization services and monitor patient responses. Collaborate with multidisciplinary teams to develop and implement effective discharge plans. Educate patients and families on health management, treatment plans, and preventative care. Maintain accurate patient records and documentation in compliance with healthcare regulations. Participate in quality improvement initiatives to enhance patient outcomes. Requirements Valid nursing license in the state of practice. Experience in Home health preferred. Proficient in phlebotomy techniques and catheterization procedures. Strong understanding of infection control practices and systems within healthcare settings. Excellent communication skills for effective collaboration with patients, families, and healthcare teams. Ability to work independently while managing multiple patient needs efficiently. Commitment to providing compassionate care and advocating for patient well-being.
