As a vital member of our care team, you will provide direct observation and support to our patients, ensuring their safety and well-being while promoting a positive and inclusive environment.

Responsibilities:

? Provide one-on-one continuous and direct observation and support to our residents ensuring their safety and well-being ? Monitor and report any changes in patient behavior, condition, or vital signs to nursing staff ? Assist patients with daily living activities ? Assist with non-clinical, ancillary tasks when not actively observing patients ? Maintain accurate and timely documentation of patient observations and care activities ? Develop and maintain positive relationships with residents, families, and interdisciplinary care teams ? Participate in team meetings and contribute to the development of patient care plans ? Maintain confidentiality and adhere to HIPAA guidelines

Requirements:

? High school diploma or equivalent preferred but not required ? Previous experience working with the elderly population preferred but not required ? Excellent communication, interpersonal, and observational skills ?
