  • Collaborates with nursing and interdisciplinary teams to support safe, patient-centered care.
Conducts environmental and room safety checks, identifies and mitigates hazards, performs frequent or constant patient observations as directed, documents findings per protocol, manages patient belongings/restricted items per policy, and supports patient care plans by responding to needs and encouraging participation in treatment activities.
ESSENTIAL FUNCTIONS
  • Work with departmental and healthcare teams to complete assignments.
  • Maintain a clean, comfortable, and safe environment for patients and visitors.
  • Identify and remove or arrange removal of immediate safety hazards.
  • Prioritize and reprioritize work based on changing patient and unit needs.
  • Complete and document environmental safety checklist for each patient room.
  • Perform patient checks and document patient whereabouts and behavior as directed (constant or frequent safety checks).
  • Assist with managing patient belongings and restricted items per nursing direction.
  • Respond to patient and visitor needs and refer concerns to nursing staff.
  • Provide input and behavioral observations to contribute to patient care planning and risk mitigation.
  • Deliver/collect meal trays and monitor intake as requested; assist with menu completion if needed.
  • Provide suicide observation when required to ensure patient safety.
  • Promote and maintain a calm, respectful therapeutic milieu; encourage patient participation in therapy and social interaction.
  • Assist with patient fresh air breaks and attend to needs such as nutritional replenishment, toiletries, linen retrieval, and phone charging.
  • Ensure nursing inspects all visitor-brought items for patient safety.

FUNCTIONAL DEMANDS PHYSICAL REQUIREMENTS
  • Exerts 20 to 50 pounds occasionally, 10 to 25 pounds frequently, or up to 10 pounds constantly to move objects.
  • Frequently reaches and handles; occasionally stoops and kneels.
BEHAVIORAL ATTRIBUTES
  • Achievement motivation, concern for order, flexibility, initiative, self-confidence, customer service orientation, interpersonal effectiveness, teamwork, analytical thinking, and information seeking.
QUALIFICATIONS
  • High school graduate or equivalent preferred.
  • Patient care, mental/behavioral health, or security experience preferred; limited hands-on care required.
  • AHA BLS CPR
    required or to be obtained within 60 days of hire.
  • DMH-approved hands-on de-escalation training within 30 days of hire for DMH-certified units and within 60 days for other locations.
