Our mission is to " bring healthcare home" by combining excellent clinical care with smart technology and strong relationships. We're looking for a Sales & Care Coordination Liaison who understands that healthcare relationships are built through both persistence and partnership .
Compensation:
$70 k base plus performance pay. Performance Incentives can be substantial, we will pay you for every patient you bring in. $20,000 to $30,000 in incentive pay is very possible but the first 3-6 months will likely see lower amounts as you ramp up your relationships About Us Thrive Healthcare is a rapidly growing, technology-focused home health agency in Los Angeles. We're looking for a Sales & Care Coordination Liaison who understands that healthcare relationships are built through both persistence and partnership . What You'll DoSales, Outreach & Relationship Building Proactively identify and develop referral sources through cold calls, in-person visits, and follow-up outreach Navigate hospital and physician office structures to find the right decision-makers (case managers, nurses, administrators) Be persistent and professional in reaching busy healthcare contacts Build long-term referral relationships through consistency, reliability, and trust
The Two-Way Street:
Beyond the Referral This is a bidirectional role , but it requires legitimate sales skills to succeed. Getting access to decision-makers takes effort, persistence, and follow-through. Once a relationship is established, you will serve as the ongoing liaison by:
Continuous Coordination:
Acting as the primary point of contact for case managers and referral partners, providing updates and ensuring expectations are met after discharge
Paperwork & Documentation Follow-Up:
Persistently following up with office staff and nurses to obtain Physician Orders and Plans of Care (POCs). You understand that signatures don't arrive automatically—you earn them through relationships and follow-up Closing the
Communication Loop:
Proactively notifying referral partners when patient needs change or re-hospitalization occurs, keeping them informed and confident in our partnership
Clinical Intake & Vetting:
Gathering sufficient clinical detail at intake and coordinating directly with our Director of Nursing to ensure each referral is appropriate and staffable This role requires comfort with outbound outreach, persistence, and accountability , paired with the ability to maintain collaborative, trusted relationships once access is earned. Technology + Patient Care Focus We are a technology-driven agency and continuously improve our systems and workflows. You will work directly with the owner of the agency , who leads marketing and technology initiatives. You don't need to be technical, but you must be comfortable learning new software and adapting in a fast-moving environment.

What We're Looking For Proven experience in sales, healthcare sales, home health, intake coordination, or hospital liaison roles Comfortable with cold calling, follow-up, and overcoming gatekeepers Strong relationship-building and communication skills Organized, responsive, and persistent Motivated by outcomes and long-term relationships Clinical license not required.
