Job Description

Position Summary As a key driver of growth, this role is responsible for identifying, engaging, and onboarding new clients within the Sonoma County region. This position is heavily focused on client recruitment and community outreach, with accountability for generating new client hours and supporting regional expansion. This is a performance-driven role with a primary emphasis on building referral pipelines and converting inquiries into active clients. The position also supports client intake and onboarding to ensure a smooth, compliant start of care. This role is designed to evolve as the region grows, with potential for expanded responsibilities or role transition based on performance and business needs.

Paid Orientation, Training, Sick Time & PTO 401(k) Plan Referral Program Core Responsibilities Client Recruitment & Community Outreach (70%) Actively identify, pursue, and convert new client opportunities within the Sonoma County region Build and maintain strong relationships with referral sources, community partners, healthcare providers, and local organizations Develop and implement strategic outreach plans aligned with marketing initiatives to maximize client acquisition efforts. Represent the organization at networking events, community meetings, and outreach initiatives Execute and track marketing and outreach strategies to drive consistent client growth Manage and maintain a pipeline of prospective clients, including follow-up, conversion tracking, and relationship nurturing Serve as the primary point of contact for new inquiries, responding promptly and guiding potential clients through initial engagement Monitor and report on recruitment activity, referral trends, and conversion outcomes Meet or exceed defined goals related to new client onboarding & client hours generated Maintain visibility into pipeline activity and provide regular updates on growth progress Participate in performance reviews, goal setting, and growth planning Identify opportunities for process improvement and regional expansion Client Intake, Documentation & Transition (30%) Conduct initial client consultations and needs assessments (in-home or virtual as appropriate) Educate clients and families on available services, service structure, and onboarding process Complete all intake documentation accurately, ensuring compliance with company standards and regulatory requirements Maintain accurate and timely records of all client interactions, assessments, and onboarding activities Ensure all documentation aligns with home care regulations, company policies, and confidentiality standards Facilitate a smooth and timely onboarding process, ensuring readiness for service initiation Partner closely with Scheduling and Care Coordinators to transition clients into active service Communicate client needs, preferences, and expectations clearly to the care team to support continuity of care Provide initial follow-up after service start to ensure a positive client experience and successful handoff Qualifications Degree in Marketing, Business, Healthcare, or a related field (Preferred). 2+ years of experience in client recruitment, sales, intake, or healthcare coordination Strong communication and relationship-building skills Ability to work independently while collaborating across teams Highly organized with strong attention to detail and follow-through Valid driver's license and reliable transportation Proficiency in Google Workspace (Docs, Sheets, Calendar)
