While this position may be advertised through John Scott House Rehabilitation & Nursing Center, it is a corporate role with BaneCare Management , supporting multiple healthcare centers and service lines throughout Eastern Massachusetts. This field-based position is not assigned to a single facility and requires regular travel to hospitals, healthcare providers, and referral sources throughout the Greater Boston region. The Clinical Liaison will build and maintain strong referral relationships, educate healthcare professionals on available services, facilitate seamless patient transitions across the continuum of care, and support the growth of BaneCare's skilled nursing, rehabilitation, hospice, and senior care programs. The ideal candidate is a licensed healthcare professional with strong clinical knowledge, exceptional communication skills, and experience in acute care, post-acute care, rehabilitation, hospice, home health, or long-term care settings. Key Responsibilities Build and maintain relationships with hospitals, physicians, case managers, discharge planners, skilled nursing facilities, and other referral sources. Conduct patient assessments to determine eligibility and appropriateness for services. Coordinate admissions and facilitate smooth transitions of care. Educate referral sources, patients, and families regarding available programs and services. Collaborate with clinical and operational teams to ensure timely and accurate admissions. Monitor referral activity and maintain accurate documentation in CRM and EMR systems. Attend community events, networking opportunities, and professional meetings to promote organizational services. Analyze market trends and identify opportunities for growth within the assigned territory. Ensure compliance with all federal, state, and organizational policies and regulations.
Qualifications Required:
Current RN or LPN preferred, other education and experience considered. Minimum of 1 year of experience in healthcare marketing, business development, admissions, case management, or clinical liaison roles. Strong understanding of healthcare reimbursement, referral processes, and care transitions. Valid driver's license and ability to travel throughout the Greater Boston area.
Preferred:
Experience in rehabilitation, home health, hospice, skilled nursing, long-term acute care, or post-acute care services. Existing relationships with Boston-area healthcare providers and referral sources. Knowledge of Medicare, Medicaid, and commercial insurance requirements.
