Opening Statement At COA of Southwestern Ohio, Care Management Supervisors lead teams that help older adults live safely and independently. You'll guide, coach, and support staff delivering critical services—while enjoying autonomy, meaningful leadership work, and the ability to maintain balance in your own life. Job Summary This role provides supervision, performance oversight, and coaching for care management staff while ensuring compliance with program standards and contracts. You'll lead with clarity and purpose—supporting both your team's success and the delivery of high-quality, person-centered services that make a measurable impact in the community. Responsibilities Leadership and Team Development Lead, coach, and develop care management staff through mentorship, onboarding, and performance feedback Foster an inclusive, supportive, and accountable team environment Communicate clear expectations, organizational priorities, and policy updates Resolve conflicts constructively while promoting collaboration and trust Delegate effectively to support professional growth and team success Performance and Program Management Monitor productivity, compliance, and quality metrics using program data and reports Evaluate performance, support goal-setting, and ensure accountability Lead or support projects aligned with program and organizational goals Analyze reports to identify trends, gaps, and improvement opportunities Address operational challenges through data-informed problem solving Service Delivery and Client Advocacy Ensure services are delivered effectively, efficiently, and aligned with client needs Monitor service authorizations and cost controls, ensuring compliance with guidelines Advocate for clients and caregivers across service providers and systems Participate in case conferences and collaborate to develop appropriate care plans Compliance, Ethics, and Policy Ensure documentation and service plans meet COA and contract standards Monitor adherence to Medicare, Medicaid, and other funding requirements Uphold and enforce organizational policies and ethical standards Maintain open, timely communication with team members and stakeholders Qualifications RN or LSW in good standing required Minimum 5 years of clinical experience in Home and Community-Based Services At least 2 years of supervisory or leadership experience Bachelor's degree in Nursing, Social Work, Gerontology, Psychology, or related field (Master's preferred for some programs) Strong knowledge of aging populations, disability services, and community resources Excellent communication, coaching, and conflict resolution skills Demonstrated ability to lead teams, manage performance, and solve complex problems Ability to work collaboratively across teams and systems Valid driver's license and ability to travel within the service area Why Join COA? 50+ years of trusted service supporting older adults in Southwestern Ohio 80%+ of employees would recommend COA as an employer Leadership role with meaningful impact on both staff and community outcomes Opportunities to grow as a leader without the burnout of traditional healthcare settings "I'm able to support my team, advocate for clients, and still have the flexibility to be present in my own life.
