Job Description

About Us:
True Heart Care LLC provides 24/7 Residential Habilitation and Support (RHS) services to individuals with intellectual and developmental disabilities (IDD). Our mission is to deliver compassionate, person-centered care that empowers individuals to live independently and thrive in their communities. We specialize in Home and Community-Based Services (HCBS) through the Indiana Medicaid Waiver program.
Position Overview:
We are seeking a Client Development Contractor with strong community connections to help expand our agency and connect us with individuals and families in need of RHS and other waiver services. This contract position offers the potential to transition into a full-time role once a set number of clients have been enrolled.
Responsibilities:
Build and maintain referral relationships with families, case managers, and community partners. Identify and connect potential clients with True Heart Care LLC. Support agency growth through outreach, marketing, and networking efforts.
Qualifications:
Established connections within the disability services or healthcare community. Strong communication and relationship-building skills. Prior experience in social services, healthcare, or case management preferred.
Contract Type:
Independent Contractor (1099), with potential for full-time employment.
Pay:
$28 - $40.00 per hour
Expected Hours:
Approximately 10 hours per week
