The Client Lead Management Specialist is responsible for managing client leads, overseeing client-related concerns, and ensuring high-quality service delivery and satisfaction. This role serves as both a relationship manager and problem solver, working closely with clients, families, caregivers, and referral partners. This position requires both office-based coordination and field work, meeting clients where they are including hospitals, skilled nursing facilities (SNF), rehabilitation centers, senior living communities, and private homes to support client intake, service transitions, and issue resolution.
Key Responsibilities:
Serve as the primary point of contact for all client concerns, service issues, and escalations Manage incoming client, ensuring timely follow-up, assessment, and conversion to active services Conduct in-person visits in the field (hospitals, SNFs, rehab facilities, senior communities, and client homes) to support client onboarding and relationship building Coordinate with scheduling staff to ensure proper caregiver-client matching and consistent coverage Collaborate with caregivers to address performance concerns, service gaps, and client feedback Handle and resolve client complaints with urgency, professionalism, and empathy Support onboarding of new clients, including care plan coordination and service setup Work with clients receiving VA benefits, ensuring proper coordination, documentation, and compliance Maintain accurate documentation of all client interactions, visits, and resolutions Monitor client satisfaction through regular follow-ups (phone and in-person) Identify trends in client concerns and recommend operational improvements Partner with referral sources (hospitals, social workers, discharge planners) to strengthen relationships and drive growth Support client retention through proactive relationship management
Qualifications:
Bachelor's degree required (Healthcare Administration, Social Work, Business, or related field preferred) 2-3+ years of experience in home care, healthcare coordination, case management, or client relations Social work background is a strong plus Experience working in or with hospitals, SNFs, rehab centers, or senior living communities preferred Strong understanding of caregiver scheduling and home care operations Excellent conflict resolution and problem-solving skills Strong interpersonal and communication skills Ability to work independently in both office and field environments Proficient in Microsoft Office and home care software systems

Pay:
$27.00 - $31.00 per hour
