Two 6-Bed Congregate Living Health Facilities (CLHFs) Position Overview We are seeking an experienced, motivated, and results-driven Admissions / Marketing / Referral Management Coordinator to join our growing team. This position is responsible for developing referral sources, managing admissions, coordinating with hospital case managers and discharge planners, and promoting our specialized Congregate Living Health Facility services throughout the Antelope Valley and surrounding areas. The ideal candidate is a self-starter who understands the CLHF admission process, responds quickly to referral opportunities, and is comfortable building and maintaining relationships with healthcare professionals in hospitals, rehabilitation facilities, and other healthcare settings. Key Responsibilities Manage all aspects of the admissions and referral process for two CLHF facilities. Review referrals and clinical documentation to determine admission eligibility. Respond promptly to hospital case managers, discharge planners, social workers, and referral sources. Build and maintain strong relationships with local hospitals, case management departments, rehabilitation facilities, and healthcare providers. Conduct regular field marketing visits to referral sources throughout the region. Coordinate facility tours and provide education regarding CLHF services and admission criteria. Collaborate with facility administrators and clinical staff to facilitate smooth admissions and transitions of care. Track referral activity, admissions, and marketing efforts. Assist with census development and occupancy goals. Qualifications Experience working in a Congregate Living Health Facility (CLHF), Skilled Nursing Facility (SNF), Subacute, Acute Care Hospital, Case Management, Admissions, or Healthcare Marketing role. Strong understanding of CLHF admission criteria and referral processes. Established relationships with local hospital case managers, discharge planners, and healthcare referral sources preferred. Ability to review and interpret clinical records and referral documentation. Excellent communication, relationship-building, and negotiation skills. Highly organized, responsive, and able to work independently. Reliable transportation and willingness to travel locally for marketing activities.
