Job Description

Header Overview Join our dynamic healthcare team as a Master Social Worker, where you will play a vital role in delivering comprehensive psychosocial support to diverse patient populations. This position offers an exciting opportunity to utilize your clinical expertise, foster meaningful patient relationships, and contribute to holistic care plans. You will collaborate with multidisciplinary teams to ensure patients and their families receive compassionate, effective, and personalized social work services across various settings including hospitals, outpatient clinics, and community-based programs. Your dedication will directly impact patient well-being, recovery, and quality of life. Responsibilities Conduct thorough patient assessments to identify psychosocial needs, including mental health status, family dynamics, and social determinants of health. Provide individual and group therapy sessions utilizing evidence-based practices such as cognitive behavioral therapy (CBT), motivational interviewing, and behavioral therapy techniques. Develop tailored care plans that incorporate clinical counseling, case management, and resource linkage to support patients' medical and emotional needs. Facilitate crisis intervention and manage complex cases involving trauma, PTSD, addiction, dementia care, or developmental disabilities. Document all patient interactions accurately within electronic medical record (EMR) systems while adhering to HIPAA regulations for confidentiality and privacy. Collaborate with medical teams to coordinate discharge planning, post-acute care, hospice services, or assisted living arrangements. Collect data for clinical research initiatives aimed at improving behavioral health interventions and patient outcomes. Experience Proven experience in clinical social work within hospital settings or outpatient clinics with a focus on mental health counseling or behavioral health services. Demonstrated expertise in child & family counseling, addiction counseling, hospice care, or working with individuals with disabilities such as autism or dementia. Familiarity with medical terminology, ICD coding (ICD-9/10), CPT coding for billing purposes, and medical documentation standards. Hands-on experience with crisis management techniques and working effectively in inpatient or nursing home environments. Strong knowledge of EMR systems for data collection and documentation; experience with home & community care is a plus. Ability to perform diagnostic evaluations using tools like ICD coding and ICD classifications; experience in research or clinical research is advantageous. Excellent communication skills combined with a compassionate approach to working with diverse populations across pediatric to geriatric age groups. Pay:
$55,189.58 - $120,000.00 per year
