Job Description

We are seeking a driven and strategic Growth Strategist for Skilled Home Health to join our team in the St. Louis market. The successful candidate will build a true growth engine within the traditional Medicare space.
Job Responsibilities:
Expand and strengthen Medicare referral networks across hospitals, skilled nursing facilities, physician offices, and senior living communities, while positioning our agency as a preferred partner in an increasingly value-based healthcare environment Identify opportunities Develop territory strategies Drive skilled census growth of traditional Medicare beneficiaries The ideal candidate understands that home health in 2026 is no longer about volume alone, but about partnerships, and sustainable, data-driven growth. Success in this role requires a consultative approach, working closely with clinical leadership to align with growth initiatives, while building meaningful, long-term relationships that reduce readmissions and improve patient care. We are looking for someone who goes beyond dropping off brochures and instead builds referral ecosystems. A strong understanding of PDGM, case mix index (CMI), and traditional Medicare dynamics is essential, along with the ability to navigate complex health systems and physician networks. Candidates should have a proven track record of growing traditional Medicare census and at least two years of experience in home health, hospice, or post-acute sales This is an opportunity to join a forward-thinking organization that is investing in modern home health growth strategy.
