What You'll Do Build and grow referral relationships with: Physician offices Hospital discharge planners & case managers Skilled nursing facilities (SNFs) Assisted living communities Educate providers on home health eligibility and our services Generate qualified referrals that convert to admissions Conduct in-person visits and ongoing follow-ups with referral sources Track outreach, referrals, and conversion outcomes Work closely with intake and clinical teams to ensure fast and smooth patient admissions What We're Looking For Experience in home health, hospice, or healthcare marketing preferred Strong understanding of Medicare home health eligibility (or willingness to learn quickly) Existing relationships in the Lehigh Valley is a strong plus Highly motivated, independent, and results-driven Strong communication and relationship-building skills Reliable transportation for local travel Compensation Structure Competitive hourly rate (based on experience) Per-Admitted Patient Bonus (Uncapped) Standard admission: $75 - $100 High-value cases (e.g., wound care, multiple disciplines, higher utilization): $125-$175 Monthly performance incentives for consistent admissions Bonuses are paid on confirmed admissions , not just referrals—rewarding quality over volume.
