Drive Quality Care Forward as a Field Supervisor!!! Acara Healthcare is looking for a Part Time Field Supervisor who will work closely with the Care Coordinator, clients and Home Care Aides within an assigned caseload to ensure clients are receiving approved services according to the Plan of Care.
Summary:
A Field Supervisor conducts in-home visits and communicates careplanning requirements, program rules, and expectations to clients, caregivers, and families. This role is responsible for supervising and training Home Care Caregivers, while proactively identifying, monitoring, and reporting changes in client conditions in support of the organization's Integrated Care approach. Duties and Responsibilities Conduct initial client in-home assessments Perform routine supervisory visits to ensure services align with care plans, regulatory requirements, and client expectations. Orient both clients and caregivers on care plans, service expectations, and applicable state guidelines. Supervise and monitor caregivers in the field to ensure quality care delivery and adherence to individualized service plans. Train, evaluate, and assess caregiver competency, including hands-on task demonstrations and performance feedback. Provide instruction and oversight on Electronic Visit Verification (EVV) processes to ensure accurate timekeeping and compliance. Monitor caregiver performance and provide ongoing coaching, support, and corrective guidance as needed. Identify, address, and resolve client and caregiver concerns or conflicts in a timely and professional manner. Validate that service plan tasks meet client needs and are not duplicated with other services or providers. Provide direct client care when necessary to prevent service interruptions and maintain continuity of care. Maintain accurate, timely, and compliant documentation for client services, supervisory visits, and required reports. Coordinate care by managing schedules, completing required forms, and communicating with case workers and internal staff. Monitor and track client status, including hospitalizations, service interruptions, and overall service delivery. Ensure compliance with Medicaid guidelines, state regulations, company policies, and HIPAA requirements. Report, document, and escalate client concerns, incidents, and service issues while supporting audits, surveys, and agency initiatives.
Requirements:
Minimum of 1 years of experience in home care, healthcare, or a related field. Valid State Driver's License with good driving record. Knowledge of Personal Care Services (Medicaid) and applicable state and federal regulations. Strong interpersonal, communication, and organizational skills. Ability to assess client needs, supervise staff, and manage multiple priorities effectively. Proficiency in documentation and use of electronic systems (e.g., EVV or similar platforms). Ability to read and write English. Bilingual in Spanish is required.
Preferred Skills:
Prior supervisory or leadership experience in home care or a healthcare setting. Experience with care coordination and interdisciplinary team collaboration. Basic computer skills preferred.
Working Conditions:
Works in office areas as well as in client homes and in assisted and independent living facilities. Interacts with clients, family members, staff, visitors, government agencies/personnel, etc., under all conditions/circumstances. Is subject to hostile and emotionally upset clients, family members, staff, visitors, etc. under all conditions/circumstances. May be exposed to infectious waste, diseases, conditions, blood or blood borne pathogens and will require OSHA training. Communicate with medical staff, nursing staff, and other unit supervisors. Maintains a liaison with residents, their families, support staff, etc. to assure that the residents' needs are continually met.
