We are seeking a dynamic Sales Intake and Community Relations Representative to join our team at SYNERGY HomeCare in Escondido, CA . As a representative of our agency, you will play a vital role in growing our client base and strengthening our presence in the community. This role combines sales, intake coordination, in-home assessments, and referral relationship building to provide exceptional care to our clients. About the Role As a Sales Intake and Community Relations Representative , you will be responsible for:
Lead Follow-Up:
Respond to new incoming leads, call within minutes, and maintain consistent follow-up until contact is made.
Lead Nurturing:
Build rapport with families, understand care needs, and guide them through the decision-making process.
Sales Conversion:
Clearly explain services, pricing, schedules, and next steps to turn inquiries into active clients.
Home Assessments:
Conduct in-person assessments to evaluate needs, safety, home environment, and care preferences.
Care Plan Development:
Collaborate with the office team to create personalized care plans based on assessment findings.
Client Enrollment:
Complete service agreements, intake forms, and ensure a smooth transition into care.
Family Communication:
Provide clear explanations, answer questions, and build trust with families throughout the onboarding process.
Referral Source Visits:
Build and maintain relationships with hospitals, SNFs, ALFs, home health, hospice, and physicians.
Community Networking:
Attend senior fairs, chamber events, and community programs to increase brand visibility.
Brand Representation:
Represent SYNERGY HomeCare professionally and compassionately in all community interactions.
Territory Management:
Plan weekly routes and maintain a consistent presence in the territory.
CRM Documentation:
Track all leads, follow-ups, assessments, and conversions accurately.
Pipeline Management:
Maintain an organized, prioritized list of active leads and referral opportunities.
Weekly Reporting:
Provide updates on leads, conversions, outreach, and assessments to leadership. Qualifications Experience in home care, senior services, healthcare sales, or intake coordination preferred Strong communication and relationship-building skills Confident conducting in-home assessments and speaking with families Organized, persistent, and comfortable with fast-paced follow-up Professional appearance and reliable transportation Warm, compassionate, and mission-driven personality Compensation & Benefits $21
