Job Description

For more than 55 years, Family Health Centers of San Diego's (FHCSD) mission has been to provide caring, affordable, high-quality health care and supportive services to everyone, with a special commitment to uninsured, low-income and medically underserved persons. FHCSD is one of the top 10 largest federally qualified health centers (FQHCs) in the country. We operate more than 90 sites across San Diego County, including 29 primary care clinics, 23 behavioral health facilities, 10 physical rehabilitation clinics, nine dental clinics, five vision clinics, four outpatient substance use treatment programs, three mobile medical units, two mobile counseling centers, two urgent care centers, and a pharmacy. Our staff provides care to over 227,000 patients each year, of whom 91% are low-income and 29% are uninsured. FHCSD provides care to all. Services include, but are not limited to adult care, chronic disease management, pediatrics, comprehensive women's care including obstetrics, dental, vision, case management, physical rehabilitation, speech therapy for children, vaccinations, infectious diseases, behavioral health, substance use counseling and a host of specialty services including cardiology, podiatry, endocrinology, dermatology, among others. FHCSD also offers supportive services to those who are unsheltered and in need of intensive case management. The breadth of our clinic locations, services and programs has grown over the last five decades, making us the largest community clinic provider of health care to the uninsured in the county and one of the top 10 largest community clinic organizations in the nation. We are also the largest health care safety-net provider, largest school-based health care provider and the largest mental health provider in the San Diego region. Job Roles Promote the PACE Program marketing plan in achieving referral and conversion goals. Develop, manage, and maintain qualified leads through referrals, telephone canvassing, face-to-face meetings, cold calling, email and networking. Conduct research in developing new referral channels, prospects, and qualified leads. Create and conduct effective presentations. Develop and maintain communication documentation to reflect follow-through efforts with referral sources and differentiating referral sources. Develop strong business relationships with referral sources to effectively educate on the PACE Program services offered using technical selling skills and in-depth program information. Identify the needs of each potential PACE Participant in aligning to the values and services that the PACE Program provides. Demonstrate an empathetic understanding based on strong listening skills. Participate and contribute to the development of the PACE Program related educational programs offered to referral sources, potential PACE Participants and their families. Ensure accurate records are maintained of all marketing and prospecting activities to include calls, presentations, closed transactions, and follow-up. Report outcomes, successes, and failed attempts on a weekly basis. Collaborate with members of the Interdisciplinary Team (IDT) and enrollment team as necessary during the enrollment process. Lead tours of the PACE Center for potential participants, family members, and referral sources. Perform other duties as assigned.
Requirements:
Bachelor's degree in human services, business, marketing or a related field required. Or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements. Traveling between sites and other locations is required as an essential function of the job. Must have a car, a valid California driver's license, and proof of minimum levels of car insurance as required under California law, although limits of $100,000 are recommended. An acceptable driving record is also required. California law requires all drivers to obtain a valid California driver's license within ten days of establishing residency. Mileage and other reimbursement governed by policy. Experience/Specialized skills (including Language) 3 - 5 years minimum experience related to proven successful sales and marketing. 3 - 5 years minimum experience related to the senior adult population, the healthcare field, healthcare management, managed care or nonprofits; or equivalent combination of education and experience that provide the skills, knowledge and ability to perform the essential job duties. Goal oriented with strong organizational skills. Self-motivated and self-directed. Demonstrated ability to communicate in a direct, empathetic and understanding manner. Ability to work effectively with culturally, economically and educationally diverse populations. Bilingual (English/Spanish) preferred. Strong analytical, critical thinking, and problem-solving skills. Successful track record in cultivating and building strong relationships with clients, staff, and coworkers. Creative problem solver who thrives when presented with a challenge. Demonstrated ability to multi-task, prioritize, and manage time effectively. Ability to be successful with minimal guidance, be proactive, and be comfortable in handling uncertainty. Strong knowledge of managing and interpreting statistics and data for reporting purposes. Proficient in computer applications to include Microsoft Office Suite of Word, Excel, Outlook, and PowerPoint.
