Clinical Business Development Liaison Job Summary We are seeking a dedicated and compassionate Clinical Liaison to join our team at Lock Haven Rehabilitation & Senior Living, a leading provider of post-acute and long-term care services in Lock Haven, PA. As a Clinical Liaison, you will play a vital role in connecting patients with the appropriate level of care, ensuring seamless transitions and optimal outcomes. Responsibilities Establish and maintain relationships with acute care hospital staff, physicians, and other healthcare professionals to identify opportunities for collaboration and referral Conduct thorough assessments of patients' medical and social needs, identifying potential barriers to optimal care and developing personalized plans to address these needs Facilitate smooth transitions between care settings, ensuring that patients receive continuity of care and minimizing the risk of readmission Collaborate with interdisciplinary teams to develop and implement individualized care plans that address patients' physical, emotional, and social needs Provide education and support to patients and their families, empowering them to take an active role in their care Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient outcomes Requirements 2 years of experience Strong communication and interpersonal skills, with the ability to build rapport with healthcare professionals and patients Ability to work in a fast-paced environment, prioritizing multiple tasks and deadlines Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions Familiarity with Medicare and Medicaid regulations, as well as industry standards for post-acute care Work Environment Lock Haven Rehabilitation & Senior Living is a dynamic and compassionate healthcare organization dedicated to delivering exceptional care to patients in our community.
