We are seeking a dynamic and motivated Hospice Liaison to join our growing team. In this role, you will: Develop and maintain strong referral relationships with hospitals, physicians, SNFs, ALFs, and community partners Educate healthcare professionals and families about hospice services Serve as a trusted resource and advocate for patients transitioning to hospice care Collaborate closely with clinical and leadership teams to support growth initiatives
Qualifications:
