School-Based Physical Therapist (PT) Islip NY School Physical Therapist (PT) Horizon Healthcare Staffing is currently seeking a dedicated and passionate School Physical Therapist to join our team at a school in Suffolk County, NY. As a School Physical Therapist, you will play a crucial role in helping students with physical disabilities or injuries improve their mobility and physical functioning to participate fully in their educational environment. This position does home visits.

Responsibilities:

Evaluate and assess students with physical impairments to determine their individual needs and develop appropriate treatment plans. Provide direct physical therapy services to students in individual or group settings as per their Individualized Education Programs (IEPs). Collaborate with teachers, other therapists, and school staff to integrate physical therapy goals into the student's overall educational program. Monitor and document student progress, conducting periodic re-evaluations as required. Communicate with parents/guardians about their child's progress, therapy goals, and home exercises or activities to support therapy outcomes. Adapt and modify school environments to accommodate students with physical disabilities and promote their independence. Attend Individualized Education Program (IEP) meetings and contribute to the development of the student's educational goals. Participate in multidisciplinary team meetings to discuss student progress and plan interventions collaboratively

Requirements:

Valid and active New York State Physical Therapist license. Minimum of one year of experience in pediatric or school-based physical therapy preferred. Strong knowledge of pediatric physical therapy techniques and interventions. Ability to develop and implement individualized treatment plans based on student needs. Excellent communication and interpersonal skills to work effectively with students, parents, teachers, and school staff. Knowledge of relevant laws, regulations, and best practices in school-based physical therapy. Familiarity with special education and IEP processes is a plus. Compassionate and patient demeanor with a genuine passion for helping children with physical disabilities.
