The pay range for this position is listed below. Actual pay rate dependent upon experience. $22.50 - $107,120.00 Position Highlights You Belong at Avera Be part of a multidisciplinary team built with compassion and the goal of Moving Health Forward for you and our patients. Work where you matter. A Brief Overview Accountable for ensuring patient care is coordinated and managed. Promotes the use of current clinical best practice standards in order to achieve high patient satisfaction and superior clinical outcomes. Serves as an ongoing resource for the clinical care team. What you will do Serves as clinical resource for patient care and oversight for assigned care teams, assists with patient assignments and determination of patient acuity levels. Responsible for assuring the development, implementation, and updates to the plan of care, recommending additional services to meet patient needs and achieve positive outcomes. Facilitates case reviews and/or case conference meetings for coordination of patient care. Participates in discharge planning processes and coordinates the referral process to ensure care across the continuum. Makes recommendations for efficient and effective utilization of resources to achieve positive outcomes above the national norm by implementing clinical protocols during the admission visit as dictated by patient's diagnosis. Monitors Infection Control Prevention activities and works with clinicians to perform Infection Control Surveillance. Maintains competency requirements regarding regulatory requirements, provides input and assures compliance with departmental policies and procedures, scope of care, safety, environmental and infection control standards to achieve positive patient outcomes. Required Education, License/Certification, or Work Experience:
Registered Nurse (RN) - Board of Nursing An active license in the state of practice
Upon Hire Preferred Education, License/Certification, or Work Experience:
