If you are seeking a dynamic role in public service, join Chesapeake Integrated Behavioral Services Mental Health Case Management team and help connect individuals to the services and supports that promote recovery, resilience, and long-term stability! This position is responsible for assessment, service planning, care coordination and ongoing monitoring to ensure appropriate utilization of services and continuity of care.Typical tasks include:
Consumer assessment, treatment planning and monitoring Community outreach to assist consumer in maintaining treatment compliance Documentation of all services in electronic medical record Quarterly reviews of consumer progress and of active service plans in compliance with Medicaid and DBHDS requirements Coordination of consumer services with services providers, community partners and familial supports on behalf of consumer Assist with discharge planning for consumers that have been hospitalized, including linkage to community resources The City of Chesapeake offers an exceptional range of benefits. Requires a bachelor's degree in social work, counseling, psychology, or a closely related field or other degree as approved by the Virginia Department of Health Professions. A master's degree in Social Work, Counseling, or Rehabilitation is required for some assigned programs.Experience:
In addition to satisfying the vocational/educational standard, this class requires a minimum of two years of related, full-time equivalent experience.Special Certifications and Licenses:
Requires a valid driver's license and a driving record that is in compliance with City Driving Standards. Requires a Qualified Mental Health Professional (QMHP) or Qualified Intellectual Disability Professional (QIDP) certification and highest Part C standards within 6 months of hire; CPR, First Aid and medication administration certifications within 3 months of hire, Certified Substance Abuse Counselor (CSAC) certification upon hire, Case Management Modules and Intellectual Disabilities Workbook completed within thirty days of hire, Part C Early Intervention Certification within six months of hire, and/or Pre-Screening Certification within 6 weeks of hire (12 weeks for part-time staff).Special Requirements:
Employees may be expected to work hours in excess of their normally scheduled hours in response to short-term department needs and/or City-wide emergencies. Emergency operations support work and work locations may be outside of normal job duties Preferred Qualifications Strong communication skills and knowledge of a wide variety of mental health community resources.
