Position Overview At Lemon Grove Care & Rehab , we are under new leadership focused on clinical excellence, accountability, and survey readiness . We are seeking a highly skilled and detail-oriented Infection Preventionist Nurse (LVN or RN) to lead and manage our Infection Prevention & Control Program within our skilled nursing facility. This is a high-impact, high-visibility leadership role responsible for protecting resident and staff safety, ensuring regulatory compliance, and driving infection control performance across the facility. Leadership team committed to support, structure, and clinical improvement Direct impact on survey outcomes and facility quality measures Collaborative environment with strong nursing leadership support Opportunities for professional growth, certification, and advancement A culture built on accountability, respect, and continuous improvement Key Responsibilities Plan, develop, implement, and oversee the facility's Infection Prevention & Control Program in accordance with CMS, CDPH, CDC, and OSHA guidelines Conduct daily/weekly infection control rounds for surveillance, case finding, and compliance monitoring Lead outbreak investigations and response , including isolation protocols, cohorting, and reporting requirements Monitor and enforce compliance with: Hand hygiene PPE usage Standard and transmission-based precautions Aseptic technique and infection prevention practices Maintain accurate infection logs, line listings, and NHSN reporting (if applicable) Prepare monthly infection control reports and present findings to QAPI and leadership teams Ensure timely reporting of communicable diseases to public health authorities Collaborate with the Director of Nursing (DON) and interdisciplinary team on care planning and infection-related interventions Educate and train staff through ongoing in-services, onboarding, and competency validation Participate in survey preparation, mock surveys, and plan of correction implementation Oversee PPE inventory and infection control supplies , ensuring readiness at all times Support employee health programs , including exposure tracking and return-to-work clearance Assist with audits, inspections, and regulatory surveys Perform direct nursing support as needed Required Qualifications Active LVN or RN license in California (required) Minimum 1 year of SNF or long-term care experience (2+ years preferred) Strong working knowledge of: CMS F880 and infection control regulations CDPH and CDC guidelines OSHA standards and exposure protocols Prior experience in infection prevention, staff development, or QA role preferred CIC certification (or willingness to obtain within designated timeframe) Experience with EHR systems (PointClickCare preferred) Proven ability to educate, audit, and hold staff accountable Core Competencies Strong attention to detail, documentation, and audit readiness Ability to lead infection control initiatives independently Excellent communication, training, and leadership skills Highly organized with the ability to manage multiple priorities in a fast-paced SNF environment Commitment to resident safety, infection prevention, and regulatory excellence Work Conditions Full-time, on-site position May require weekend, on-call, or outbreak response availability Frequent exposure to clinical and infectious environments Must be able to assist in emergency situations, including evacuation support For questions or more information, contact Priscilla at (858) 316-4630.
