Job Description

Hilltop Healthcare and Rehabilitation Center is seeking a dedicated and detail-oriented Licensed Practical Nurse (LPN) to serve in a dual role focused on Infection Control and Staff Development. This position is responsible for overseeing the facility's infection prevention program while coordinating and delivering staff education to ensure compliance with all regulatory standards and high-quality resident care.
Responsibilities:
Infection Control:
Develop, implement, and maintain the facility's infection prevention and control program Conduct surveillance, tracking, and reporting of infections in accordance with CDC, state, and federal guidelines Perform routine audits to ensure compliance with infection control standards Provide education and guidance to staff on infection prevention practices Monitor and respond to outbreaks, implementing appropriate interventions Maintain accurate documentation and reporting logs
Staff Development:
Coordinate and conduct new hire orientation for nursing staff Plan and deliver ongoing education, in-services, and competency training Ensure staff compliance with required certifications and training Identify educational needs and develop training programs accordingly Maintain records of all training and competencies Support performance improvement initiatives through education
Qualifications:
Active LPN license in PA Previous experience in long-term care or skilled nursing required Infection control experience or IPCO certification highly preferred Strong knowledge of state and federal regulations Excellent organizational, communication, and teaching skills Ability to work independently and collaboratively
