JOB SUMMARY
Position responsible for participating in the coordination of all systems / services required for an organized, multidisciplinary team approach and assures quality, cost-effective care for the identified patient population.
MINIMUM QUALIFICATIONS
1.
Education/Specialized Training/Licensure:
Masters Degree in Behavioral Sciences or Social Work with Licensure (LPC or LMSW). An LCDC is preferred. 2.
Work Experience:
Experience in a hospital/health setting preferred Experience working with patients with chemical use/abuse/dependency counseling and treatment preferred. 3.

Computer
